We are trained and dedicated professionals licensed to provide a variety of individually tailored services and supports under the Medicaid Home and Community Based Waiver (HCBS). We work to advance the quality of life for persons with disabilities and persons age 65 and older in any county in the state, subject to the standards of the Minnesota Statutes, Chapter 245A and Minnesota Statutes, Chapter 245D.

Our quality services are flexible and responsive to individual needs and choices, while maintaining the fiscal integrity that ensures the long term stability and security that every person expects and deserves.
